The Practice offers a comprehensive range of travel advice and vaccinations. If you are travelling and want advice from our nurse advisors, please download the Travel form below, and return it to the surgery fao the Practice Nurse as early as possible. Pre-travel health and vaccination assessment form. This form is to be completed by patients going abroad on holiday or a business trip for example, who wish to have a risk assessment of their travel plans
It is advised to complete this form as much as possible and to hand it in at the surgery in order for the practice Nurse to identify what vaccinations are required before travelling. At the same time, you can also make an appointment with a Practice Nurse for your travel vaccinations.
Please be aware that you may need some vaccinations 3 months before starting your holiday!
